Helping patients access quality medical care with respect and dignity is our goal at the Community Health Services of Addison County (CHSAC) – Open Door Clinic.  As a free clinic, we provide health care for uninsured and underinsured adults.  Our clinics operate every Tuesday evening in Middlebury and every other Thursday evening in Vergennes.  We also offer occasional Friday morning appointments in Middlebury.  Appointments are needed for all clinics.


We are able to operate effectively and efficiently because of the strength of our volunteers.  Our many volunteers are the lifeblood of our clinic.  Without their abundant talents, and tireless generosity, we would be unable to serve our community.

Our dedicated staff and volunteers work together as a team to provide high-quality comprehensive care. Our compassionate providers take pride in paying close attention to each patient's needs.  All of our providers are volunteer physicians, nurse practitioners, physical therapists, chiropractors, nurses and dieticians from our local community.

TheOpen Door Clinic has written Privacy Practices, in accordance with the Federal Privacy Rule, which are available for your review.  Our Privacy Practices document describes how we may use or disclose a patient's protected health information, with whom that information may be shared, and the safeguards we have in place to protect it.  This notice also describes a patient's rights to access and amend their protected health information.  They have the right to approve or refuse the release of specific information outside of our clinic system, except when the release is required or authorized by law or regulation.

For all medical or appointment inquiries, please call our office at 388-0137.  If no one is available to answer your call, you can leave a confidential message for the nurse case manager at extension 1.  Because email is not a secure method of communication, do not email medical or appointment related questions or information.
